Cast Nephropathy
A condition linked to kidney damage in multiple myeloma
Cast nephropathy is the principal cause of severe acute kidney injury in Multiple Myeloma. In this condition, renal distal tubules are blocked by waxy deposits formed from monoclonal free light chains (FLCs) in association with Tamm-Horsfall protein (uromodulin).

A serum free light chain (FLC) value ≥500mg/L is indicative of cast nephropathy in a patient with unexplained AKI
In patients with dialysis dependent AKI, sustained reductions in serum FLCs following treatment are associated with renal recovery and improved patient survival.
Rapid diagnosis of cast nephropathy combined with early treatment is associated with improved patient outcome.
